Workers’ Compensation Claims
Refer to the states listed below and locate the state(s) you operate in or the state(s) in which the injury occurred. Download the First Report of Injury document and follow the instructions on the form. Please email a completed First Report of Injury document along with a brief message describing the claim to claims@grandriverservices.com to report a new claim. Additionally, the kits contain important posting notices for employers to place in conspicuous locations of all business locations
